INSURANCE

I am an in-network (INN) provider with Blue Cross Blue Shield (BCBS) for benefit plans: State Health Plan, PPO, Blue Value POS, Indemnity, and Blue Care HMO. I am happy to verify benefits for you, or you can contact BCBS Member Services and ask if Chancie Chaney, NPI # 1093347023, is covered under your health insurance plan.

I am an out of network provider for all other insurance providers, except Medicaid or Medicare, and my services may be covered in full or in part by your health insurance or employee benefit plan. If applicable, please contact your insurance company to ask if they reimburse for out-of-network mental health services with the billing code of 90837 or 90834 (Individual Therapy). Please check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?

  • What is my deductible and has it been met?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is authorization required?

  • Which services are covered (e.g., couples therapy, group therapy, telehealth, individual, phone sessions)?

You have the right to pay for your services out of pocket without seeking insurance reimbursement in order to avoid having a mental health or substance abuse diagnosis shared with your insurance company. Insurance coverage is never a guarantee as a company may choose to not cover services at any point of treatment for a variety of reasons for both in-network and out-of-network providers.

FEES

Session rates range depending on the service provided. Talk therapy sessions are typically 50 minutes in length and Accelerated Resolution Therapy sessions are typically 75-90 minutes. Sliding scale/reduced fees are available on a limited basis.

Fees are structured as follows:

• Initial Session / Diagnostic Evaluation $180
• Individual Psychotherapy Session $140
• Accelerated Resolution Therapy Session $175

Payment is collected during each session via check, cash (no change available), Health Savings Account, and major credit cards including AMEX.

CANCELLATIONS AND MISSED APPOINTMENTS

Please provide 24-hours notice if you must cancel an appointment. If you arrive more than 15 minutes late, you will not be able to be seen for your session. The session will be considered a late cancellation. A charge of $100 is applied when appointments are missed or cancelled with less than 24 hours notice. This is necessary because a time commitment is made to you and is held exclusively for you. Missed sessions are not billable to insurance and you will be charged the full fee as published in our fees, not your copay amount or your insurance company’s contracted amount. Emergencies are excluded from this policy.
